Template Content
This template uses four types of elements defined by the value selected in the "Structure" column. The possible values are listed below:
Section: Every row under a section corresponds to that section until a new one is found
label: The row will be treated as a label
question: The row will be treated as a question
score: The row will be treated as a score
When the user downloads the excel template will see there are many sections and right above each section the data elements that belong to this section.
To add another Section the user only will have to add a new row ( Is very important to remark that an entire row needs to be added ) right below the previous section’s last Data Element.
For a new section, the user has to select "Section" on the Structure ( B ) column.
Then write a form name that will be the name displayed for the new section.
To add a new Data Element you have to find the right position where you want it and add a new entire row right below.
For a new DE the user will have to select between a label or question
If "label" is selected by the user only will have to fill the form name
If the option selected is "question" the User will have to fill in the form name and other fields
The critical and compulsory columns by default are going to be filled with "No". If the question is compulsory indicates that the user will not be able to complete the assessment until this question has been answered. A critical question means that these questions will count for the calculated score.
The value type field is going to define what kind of response the system expects to receive from the user.
The user will be able to fill the option set with options set previously created on DHIS.
The legend field allows the user to choose if he wants to apply some colour for "correct" or "incorrect" answers. Also will be created on DHIS.
The numerator and Denominator columns will be filled with 1.
The compositive indicator Field is the order of the questions for example if your listing the First Section ( Section 1) Data Elements this will start at 1.1 and ends in 1.x.
The parent questions field is going to be the way for the user to decide the hide/show logic for some questions. The user will have to select for the parent question the value of the "Parent question" Column A and put it in the Parent questions Column L of the Child question.
The answer value field will be filled by the user with the options that expect to receive in the parent question for showing the child question.
Feedback text and descriptions fields should be filled by the user for example when you want to display some text above the DE to do some reminder or something you fill the "Feedback" and the description is for the configuration side ( Description will not be displayed on the app).
Under the "Scores" section user will be able to add a new score.
To add a new score select the position where you want to insert it and add a new row.
Select column B " Score" and fill in the form name and the compositive score.
Exporting a pre-filled template
You can always download an existing template to work on it like an excel template just have to click on "Download template" and you will be able to open it in Excel.

Importing a template
Once the user has done all the changes on the Excel file will have to save them.
Then click on the "Import template" button and select your file.
When you have selected your program will see a dialogue box with all the created, updated and deleted elements.
Click on confirm, double check that everything is as you expected and then if everything looks OK, click on validate and save to begin the set up of the program.
