The form has two sections with twelve and five questions respectively and all of them have four possible answers:
Yes – completely
Mostly
Partly
Rarely
No – not at all
N/A – not applicable

When the form is finished, tap on the save icon to complete the M&E System Assessment stage. Two options will be displayed:
Finish and complete: The app will close the stage and no further changes can be made.
Not now: The app will save the answers but remain the stage open.
